1. Please go to Home Page >> Library & Document >> Find "Clinic Library" in the dropdown menu

2. Click the "+" button on the top right >> select "Submit Survey for Quote" 

3. Upload the file from your pc, please make sure to have all your questions and answer options ready. Allowed formats: pdf and doc.
