BlueJaySNF lets you easily manage your patients. Let's take a look at how it works.

Navigate to "Manage Region & SNF" > click on the "Patients" tab > you can use the filters such as "Region" and "SNF" to locate the patient in the system > tap on the pencil icon to open a particuar patient's profile. 

On the following page, you will be able to update your patient's information, i.e., DOB, Zip Code, Name, ect. To discharge a patient, simply click on the "Discharge" button on the bottom.

To transfer a patient from one SNF to another, open the "SNF" dropdown menu > select the facility where this patient will be transfered to. Once you completed, click on the "Save" button.
